What documents are required for moving from Canada to US?

Visas are not required for U.S. citizens entering Canada from the U.S. However you need
  Proof of your U.S. citizenship such as your U.S. passport, or certified copy of your birth certificate issued by the city, county or state in the U.S.
  Photo identification, such as a current, valid driver’s license.
  If you are a dual U.S./Canadian citizen you should always present yourself as a Canadian citizen when entering Canada.
If you have a car:
  Drivers License
  Currently, motor vehicles that are less than 15 years old are required to meet or surpass Canadian safety emissions standards and bear a statement of compliance label, along with the date the vehicle was manufactured.
  If you are planning to take your automobile into Canada from the United States, It is better to contact "Registrar of Imported Vehicles". This office provides information about vehicle inspection, certification, and registration.

Are we allowed to bring money, prescription medicine, animals, plants, tobacco, and alcohol?

Well, You are allowed to bring, but consider the following factors for each category.

Money: Depending on the status of your move into Canada, you may be subject to taxes and duties for your shipment of household and personal items that exceeds $1O,OOO in total value.
Medicine: Keep the medication in its original and labeled container to avoid problems. Syringes should be accompanied by a medical certificate that shows they are for medical use and should be declared to Canadian Customs officials. It may also be wise to carry with you an extra prescription from your doctor in the event your medication is lost or stolen and to attest to your need to take such prescriptions.

Penalties for possession, use, and dealing in illegal drugs are strict in Canada. Convicted offenders can expect jail sentences and fines.
Animals: Dogs and cats from the U.S. that are at least three months old need signed and dated certificates from a veterinarian verifying that they have been vaccinated against rabies within the last three years. The certificate must clearly identify the animal. If your dogs or cats are less than three months old, you do not need a certificate of rabies vaccination signed by a veterinarian to enter Canada. However, the animals must be in good health when they arrive.
Plants: Generally house plants from the United States can enter without phytosanitary certificates or permits (House plants are defined as plants commonly recognized and grown indoors), but there are some restrictions on the entry of plants in order to control potential carriers of insects and disease. All other plants from the United States require a phytosanitary certificate from the U.S. Department of Agriculture. Plants from Hawaii must be bare-root (free of soil).
Alcohol: Normally 1.14 liters of wine or liquor, OR 24 - 12 ounce cans or bottles of beer per person (you must be over legal age). If you intend to import the contents of a bar or wine collection, you must contact the appropriate provincial or territorial Liquor Control Board authority before you ship these items so that you can pay the appropriate fees and assessments in advance. To obtain release of the shipment in Canada, you must produce proof that these duties and taxes have been paid.
Tobacco: Usually 200 cigarettes, 50 cigars or 400 grams of tobacco per person.(you must be over legal age)
